To all our members covered under Central States Health Plan:
The Patient Protection and Affordable Care Act (ACA) and the Mandatory Insurer Reporting Law require TeamCare to obtain Social Security Numbers (SSN) for all covered individuals, which includes spouses and children. TeamCare is required to issue “IRS Form 1095-B Health Coverage” beginning in 2016 for the 2015 tax year to verify that each covered individual has the required minimum essential healthcare coverage. In addition, the MIR requires that TeamCare collect and supply the government with Medicare Health Insurance Claim Numbers (HICN) if a covered spouse is receiving Medicare benefits.
Next week we will be sending the enclosed letter to those participants for whom we do not have the required SSN or HICN.
Should any of your members receive this letter, please assure them that the information is required under the Affordable Care Act and all HIPPA Privacy Act standards and safeguards will be in place.
